As a clinician and horror enthusiast, the premise of a game featuring a main character described as having &#8216;paranoid schizophrenia&#8217; (according to the game\u2019s original <a href=\"https:\/\/store.steampowered.com\/app\/946920\/PARANOID\/\" target=\"_blank\" rel=\"noopener\">Steam<\/a> listing, now revised) immediately intrigued me.<\/p>\n<p>My hope, albeit low, was for the game to approach the protagonist\u2019s paranoia \u2014 depending on the diagnosis, paranoid personality disorder (PPD) affects <a href=\"https:\/\/www.merckmanuals.com\/professional\/psychiatric-disorders\/personality-disorders\/paranoid-personality-disorder-ppd#:~:text=The%20estimated%20median%20prevalence%20is,of%20increased%20prevalence%20in%20families.\" target=\"_blank\" rel=\"noopener\">3.2% to 4.4% of the general population<\/a>, while paranoid schizophrenia impacts just 1% \u2014with nuance or even grace, rather than centering the horror solely around the character\u2019s supposed mental illness. But who am I kidding? This is the same studio that brought us the nightmarish <em>Agony<\/em> and its baby-drinking spinoff, <em>Succubus<\/em>. One day, Patrick receives a call from his sister, who has been missing for over a decade, announcing her impending visit. What follows is a journey to uncover the truth in an attempt to save his sister and his sanity.<\/p>\n<p>Full disclosure: At launch, this game was a buggy mess. When I tried playing it on my Steam Deck, I found myself in Patrick\u2019s apartment, unclear on where I was or what to do. At first, I thought this was intentional; a game design that aims to instill confusion in the player. Nope.<\/p>\n<p>It turns out many players experienced this \u2014 being dropped into the middle section of the game, clueless about what was happening or how to progress. Deleting game data and rebooting didn\u2019t fix it, and when <a href=\"https:\/\/store.steampowered.com\/news\/app\/946920\/view\/3906373844372334829\" target=\"_blank\" rel=\"noopener\">MadMind issued a patch<\/a>, some players reported game crashes.<\/p>\n<h3>Missed Opportunities<\/h3>\n<p>Once I got my bearings, a few story elements intrigued me. For instance, Patrick\u2019s ally, Detective Ortega, who aids him in the search for his sister. The notion of a psychotic protagonist collaborating with an investigator to discover the truth was an interesting narrative beat. It subtly subverts the notion of a helpless mentally ill individual, offering a more realistic representation of resilience.<\/p>\n<p>However, some story elements remained unclear. There is a moment midway through the game when Patrick calls for his sister from his apartment balcony. The character seemed to reference something happening below, but no matter how much I turned the camera, I couldn\u2019t determine what Patrick was screaming at.<\/p>\n<p>The game offers two narrative paths with similar endings, with both linking <em>Paranoid<\/em> to MadMind\u2019s previous title, <em>Succubus<\/em>. Yet, I found this narrative connection puzzling. Was Patrick\u2019s sister kidnapped by a cult to summon the demon Ishtar from the previous game? Why is Patrick compelled to cut out people\u2019s hearts and feed them to a faceless mouth demon? Is Patrick\u2019s enigmatic father behind it all? Could Patrick\u2019s voyeuristic neighbor secretly be a succubus? <span class=\"Apple-converted-space\">I have no idea because the story was plagued by bugs, literally, and poor game design, leading to my next point.<\/span><\/p>\n<h3 class=\"p1\">GAMEPLAY &amp; DESIGN<\/h3>\n<p><em>Paranoid<\/em> emphasizes puzzles and exploration to drive the story forward, which would have been enjoyable if not for near-constant distractions.<\/p>\n<p><a href=\"https:\/\/www.relyonhorror.com\/wp-content\/uploads\/2024\/01\/ss_3ff9a825ba2a74d4d80ca709ddd431469d5c18cf.1920x1080-e1704815410855.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-large wp-image-112310\" src=\"https:\/\/www.relyonhorror.com\/wp-content\/uploads\/2024\/01\/ss_3ff9a825ba2a74d4d80ca709ddd431469d5c18cf.1920x1080-1024x575.jpg\" alt=\"\" width=\"1024\" height=\"575\" \/><\/a><\/p>\n<h3>Exploration<\/h3>\n<p>Interacting with objects in Patrick\u2019s apartment immerses players in his isolated world. Players can watch TV, listen to a recording from Patrick\u2019s sister, spy on neighbors by removing wall panels, or crush cockroaches in Patrick&#8217;s fists. Despite seeming trivial, these elements offer a glimpse into the protagonist\u2019s perspective on his own life.<\/p>\n<p>However, I&#8217;m not particularly fond of portraying a mentally unsound central character as a pervert. The player can spend an exorbitant amount of time watching their aforementioned neighbor shower and, well\u2026 <em>do other things<\/em>.<\/p>\n<p>Voyeurism aside, these, uh, &#8220;moments&#8221; alternate with sequences where Patrick is pulled into a delusional world, often culminating in acts of violence. While these elements contribute to the psychological horror theme, MadMind&#8217;s attempts often feel flat and uninspired. An exception is Patrick\u2019s mailbox, which could use a lesson in consent.<\/p>\n<h3>Sound<\/h3>\n<p>The game\u2019s audio design, however, is its most detrimental aspect, at times rendering <em>Paranoid<\/em> nearly unplayable. Players must constantly listen to an audio track, supposedly Patrick\u2019s thoughts or him talking to himself. While this might have cleverly mirrored the actual experiences of individuals living with something called <a href=\"https:\/\/livingwithschizophreniauk.org\/information-sheets\/understanding-voice-hearing\/\" target=\"_blank\" rel=\"noopener\">persecutory voices<\/a>, it interferes with crucial story beats and gameplay instructions when not in cutscenes (an important note here: <a href=\"https:\/\/medicine.yale.edu\/news\/yale-medicine-magazine\/article\/when-researchers-listen-to-people-who-hear-voices\/\" target=\"_blank\" rel=\"noopener\">voice-hearers account for 8% of the population<\/a>).<\/p>\n<p class=\"p1\">This is most jarring in Patrick\u2019s apartment, where the player is required to explore the space to progress. Here, the clash between TV audio, his sister\u2019s recordings, and Patrick\u2019s inner voices creates a cacophony that&#8217;s enough to drive any player insane.<\/p>\n<h3>Madness<\/h3>\n<p>At various points, Patrick\u2019s mental stability can deteriorate, evident in visual filters that blur the field of vision. To counter this, players collect and regularly take medications to prevent increasing hallucinations. Taking too many, however, results in vomiting during combat, leaving Patrick vulnerable to attack.<\/p>\n<p class=\"p1\">Did I mention that medications are different from health? Though an interesting design choice, many players, including myself, frequently took pills instead of health drinks by mistake, leading to frequent and annoying vomiting spells.<\/p>\n<h3>Enemies<\/h3>\n<p style=\"text-align: center;\"><a href=\"https:\/\/www.relyonhorror.com\/wp-content\/uploads\/2024\/01\/paranoid-2.webp\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-112323\" src=\"https:\/\/www.relyonhorror.com\/wp-content\/uploads\/2024\/01\/paranoid-2.webp\" alt=\"\" width=\"830\" height=\"535\" srcset=\"https:\/\/www.relyonhorror.com\/wp-content\/uploads\/2024\/01\/paranoid-2.webp 830w, https:\/\/www.relyonhorror.com\/wp-content\/uploads\/2024\/01\/paranoid-2-300x193.webp 300w, https:\/\/www.relyonhorror.com\/wp-content\/uploads\/2024\/01\/paranoid-2-768x495.webp 768w\" sizes=\"auto, (max-width: 830px) 100vw, 830px\" \/><\/a><\/p>\n<p class=\"p1\">Other design choices are noteworthy for their poor taste. While I found some of the enemies intriguing, like the bisected woman <a href=\"https:\/\/www.relyonhorror.com\/latest-news\/the-new-paranoid-trailer-makes-me-anxious\/\">featured in early promotions<\/a>, the player will largely experience the same enemy type over and over again. Oh, and these enemies are mostly homeless people that the player must beat to death to progress. Yup.<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p class=\"p1\">This is perhaps the most insensitive design choice in <em>Paranoid<\/em>. Homelessness and mental health often intersect, especially in the United States. In recent years, <a href=\"https:\/\/www.thenation.com\/homeless-homicides-are-on-the-rise\/\" target=\"_blank\" rel=\"noopener\">violence towards unhoused persons appears to be on the rise<\/a>.<\/p>\n<p class=\"p1\">While Patrick will occasionally fight entities born of his own hallucinations, why the main character is attacking unhoused people is never clearly explained. This is to say nothing about the fact these NPCs are rendered as shouting conspiracy theories and humbling recollections of war, further conflating common enemies with unhoused combat veterans. Yikes!<\/p>\n<h3 class=\"p1\">COMBAT<\/h3>\n<p><a href=\"https:\/\/www.relyonhorror.com\/wp-content\/uploads\/2024\/01\/ParanoidScreenshot04.jpeg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-large wp-image-112325\" src=\"https:\/\/www.relyonhorror.com\/wp-content\/uploads\/2024\/01\/ParanoidScreenshot04-1024x576.jpeg\" alt=\"\" width=\"1024\" height=\"576\" srcset=\"https:\/\/www.relyonhorror.com\/wp-content\/uploads\/2024\/01\/ParanoidScreenshot04-1024x576.jpeg 1024w, https:\/\/www.relyonhorror.com\/wp-content\/uploads\/2024\/01\/ParanoidScreenshot04-300x169.jpeg 300w, https:\/\/www.relyonhorror.com\/wp-content\/uploads\/2024\/01\/ParanoidScreenshot04-768x432.jpeg 768w, https:\/\/www.relyonhorror.com\/wp-content\/uploads\/2024\/01\/ParanoidScreenshot04-1536x864.jpeg 1536w, https:\/\/www.relyonhorror.com\/wp-content\/uploads\/2024\/01\/ParanoidScreenshot04.jpeg 1920w\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" \/><\/a><\/p>\n<p class=\"p1\">Patrick employs various melee weapons in his quest to collect still-beating hearts for\u2026 reasons? Oh, yeah, to find his sister. Right.<\/p>\n<p>These include pipes, hammers, axes, and even his bare fists. In these combat sequences, the player can see that Patrick\u2019s hands are scarred from previous altercations. As players engage enemies, fresh wounds appear, two design choices I appreciate.<\/p>\n<p class=\"p1\">Executing well-timed moves results in brutal finishers, often requiring quick-time button-mashing, which is cumbersome and difficult to pull off. More often, combat feels random, with enemies slumping to the ground in a dissatisfying finish. This style became monotonous quickly; I found myself trying to avoid it, but the cramped environments often forced confrontations.<\/p>\n<h3 class=\"p1\">PUZZLES<\/h3>\n<p style=\"text-align: center;\"><a href=\"https:\/\/www.relyonhorror.com\/wp-content\/uploads\/2024\/01\/ss_77a191fe2808e47bdff532741feb00034da17c28.1920x1080-e1704815481227.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-large wp-image-112309\" src=\"https:\/\/www.relyonhorror.com\/wp-content\/uploads\/2024\/01\/ss_77a191fe2808e47bdff532741feb00034da17c28.1920x1080-1024x575.jpg\" alt=\"\" width=\"1024\" height=\"575\" \/><\/a><\/p>\n<p class=\"p1\"><em>Paranoid<\/em> prioritizes puzzles, and this is where the game comes closest to shining. Through frequent backtracking, item gathering, and ingenuity, players piece together tiny mysteries to progress the story forward. Whatever the story actually is.<\/p>\n<p class=\"p1\">One puzzle in the sewers requires players to pay attention to the cockroaches crawling along the walls. At first, I was stumped, but was pleasantly surprised when I realized the trick was to listen to the bugs. Ironically. This moment stands out as one where originality peaks through the muck and mire of an otherwise dreadful gaming experience.<\/p>\n<h3 class=\"p1\">CONCLUSION<\/h3>\n<p><a href=\"https:\/\/www.relyonhorror.com\/articles\/diagnosing-the-genre-mental-health-in-horror-gaming-pt-1\/\">As I&#8217;ve written previously<\/a>, I hold little forgiveness for game developers who propagate dangerous stereotypes about neurodiverse persons and those with lived experiences to tell lackluster stories. In this regard, <em>Paranoid<\/em> is everything I feared the game would be.<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p class=\"p1\"><a href=\"https:\/\/madmind-studio.com\/\" target=\"_blank\" rel=\"noopener\">MadMind Studio<\/a>, not afraid <a href=\"https:\/\/www.relyonhorror.com\/latest-news\/agony-developer-content-limits-should-not-exist-in-video-games\/\">to push boundaries and be controversial<\/a>, displays timidity in their latest title. Still, it&#8217;s evident that the developer prioritizes shock over substance, and relying heavily on Patrick\u2019s mental illness to justify story beats and gameplay that lacks substance is a clear continuation of that prioritization.<\/p>\n<p class=\"p1\">But let&#8217;s face it, that&#8217;s not the kind of game MadMind makes. This is to say nothing of the buggy rollout and near unplayability of portions of the game.
